RN Practice Environment

• A consultative program designed to help a SNF incorporate effective and evidence-based nursing organization structures and care delivery practices into its everyday operations. This program is intended to take a facility through a comprehensive change process to reconfigure person-centered care delivery that is guided by professional nursing. It will begin with an organizational assessment and gap analysis. Recommendations for changes in nursing organization structure and processes will be made.



• In consultation with facility staff, an action plan and timeline will be developed to guide the implementation of these changes using an implementation timeline as a guide.



• Organizational consultants and specific clinical content consultants will be available online, by telephone or in person to guide staff through the change process.
